Minor updates & bug fixes

When Pocket list is already open in another tab, Open Pocket list action (from keyboard or extension button), switches to that tab instead of opening a new one.
- If Pocket isn’t already open, the Open Pocket list action opens the list in current tab if it’s a blank tab, instead of always creating a new tab.
When the user isn’t logged in, pressing the extension button opens options page. Earlier it used to start the login process which sometimes caused bugs.
Update minimum supported Chrome version to 49 so I could use newer web technologies available.
Chrome v.49 is over a year old (current version is 59), so this should hopefully not impact many users.

The extension sometimes repeatedly requested re-login after a period of using offline.
Additional formatting on Pocket website broke sometimes (usually on page refresh), leading to AcceleReader time tags, etc looking big and ugly. Fixed.
